To the original Certificate
of Representation in Oregon Arbitration, which has been completed and signed
by the out-of-state attorney, attach the following:
- An original, current (dated within three months) Certificate of Good Standing from the Bar association or highest court in the out-of-state attorney's home jurisdiction;
- A certificate of professional liability insurance covering the out-of-state attorney's practice of law in Oregon that includes policy limits and deductible amount, OR, a copy of the notice to the client that informs the client the out-of-state attorney has no such insurance, OR, proof the out-of-state attorney is employed as in-house counsel or as a government attorney, and therefore is exempt from the requirement to carry professional liability insurance; and
- A check made payable to the Oregon State Bar in the amount of $200.

The Certificate of Representation and all required attachments should be mailed to Regulatory Services at the Oregon State Bar, P.O. Box 231935, Tigard, OR, 97281-1935. The attachments will be retained by the Bar. A copy of the Certificate of Representation must also be served upon the other parties to the arbitration and the arbitrator or arbitration service.
